Nurses require critical appraisal skills to assess the quality of evidence to safely make clinical decisions. The overarching learning objective of this assessment task is for students to critically appraise one published quantitative study, interpret the results and apply the results to the scenario provided.
By successfully completing this assessment task, you will develop and demonstrate your ability to:
Critically appraise research using a relevant appraisal framework
Critique, interpret, and apply study findings to a clinical practice scenario
Clearly explain complex concepts and scientific knowledge in writing, when managing unfamiliar problems.
There are four key questions to be asked when completing an appraisal:
1.Does this study address a clearly focused question (Links to an external site.)?
2.Did the study use valid methods to address this question?
3.Are the valid results of this study important?
4.Are these valid, important results applicable to my patient or population (scenario)?
Scenario
You are a senior nurse working in the emergency department of a small rural hospital and you are the departments’ representative on the hospital quality and safety committee. The hospital quality and safety committee has seen an increase in adverse events (cardiac arrest and / or urgent transfer to a tertiary hospital) due to late recognition of deteriorating emergency department patients. In response to this, your hospital plans to implement a standardised approach to patient assessment and monitoring to improve early recognition of the deteriorating patient. You have been asked to conduct a literature review to find the evidence-based recommendations for patient assessment and monitoring in the emergency department to assist in the recognition of deteriorating patients.
Your search of the literature finds the following article:

In your summary, include the study aim(s) and patient, intervention, comparison, outcome and time (PICOT) study elements. Specify the primary outcome identified in the study.
Critical Appraisal (950 words approximately).

appraise the validity of the study by Curtis et al (2021), with reference to appropriate literature where necessary.
In your appraisal you need to answer the following questions:
1.Is it clear in the study what is the ‘cause’ and what is the ‘effect’ (i.e. there is no confusion about which variable comes first)?
2.Were the participants included in any comparisons similar? Were the participants included in any comparisons receiving similar treatment/care, other than the exposure or intervention of interest? Was there a control group?
3.Were there multiple measurements of the outcome both pre and post the intervention/exposure?
4.Was follow up complete and if not, were differences between groups in terms of their follow up adequately described and analysed?
5.Were the outcomes of participants included in any comparisons measured in the same way? Were outcomes measured in a reliable way?
Report & Interpret the Results (200 words approximately)
Please answer only the following questions about the results of the study within your appraisal:
1.Was a target sample size identified and was this sample size met?
2.What was the difference between the pre-intervention group and the post-intervention group in the number of?
1.Patients who had a cardiac arrest,
2.Patients who had an unplanned intensive care admission,
3.Patients with Acute Condition for Escalation (PACE) tier 2 call
Report and interpret the statistical significance of these results.
3.What was the number of nursing management errors identified to be a causal factor for the patient’s deterioration for the pre-intervention group and the post-intervention group? Please report and interpret the statistical significance and confidence interval for this result.
Overall Statement of Validity (200 words approximately)
Based on the validity of the study and strength of the results, discuss the possible application of the evidence to the scenario provided. Provide an overall summary judgement statement of the validity of the study.
