ACTION PLAN FOR IDEA STUDENT ACHIEVEMENT GOAL
The primary goal is to identify and detect early signs of disabilities and support early identification and intervention. Upon achieving this goal each child will be educated according to individual need to ensure academic success.
SUPPORT DATA Process and Procedures Community Support
Students will be given an evaluation assessment and diagnostic equipment. They will be tested for developmental delay in cognitive, physical,
communication, social or emotional development.
Once they have been diagnosed, they will be offered the appropriate services to help with improvement. Such as family training
and counseling, home visits, speech or language pathology and audiology therapy,
occupational and physical therapy, and health services.
IFSP will be used every six months to test improvement. This will determine proper
educational placement.
If there aren’t any community involvement as it pertains disability involvement I will go out into communities and find ways of implementation. Be sure that parents
take advantage of the recreational meets, gatherings and other activities designed for disabled individuals. To be sure that parents take part at least once or twice there
will be some sort assignment centered around the community related activity.
STRATEGY DESCRIPTION The overall common goal is to be sure that each student progress academically. Based on the assessment provided students will be placed in settings accordingly. Each child will be required to attend at least one disabled community activity. For support for the parents in those communities where these actives are scarce, I will create my own. I will find creative ways to enter the community and bring awareness to those of disabled. Parents will also receive frequent updates on their child improvement rate.
